We therefore ask you to complete a medical history form every 12 months.
It is helpful to bring along a note of any medication you are taking when you come for an appointment. The dentist will also verbally check that your medical history hasn’t changed at every visit.
You can also bring a copy of your current prescription at your visit; we will be happy to scan/document the medication you are taking.
Patient confidentiality is taken seriously, and all information about our patients is treated in the strictest confidence in accordance with our practice policy.
